The Request for Service and Suggestion Form is a way to contact the City of Longmont directly. Citizens, or people who work in or are visiting our City are encouraged to use this form to comment on any issues or report any concerns.

Please note that all information in City databases is subject to the Open Records Act, including information submitted via this form. E-mails are only checked once per day, Monday-Friday. For an emergency, please dial 9-1-1 or Longmont Dispatch at (303) 651-8501. For other urgent requests that require assistance, please call the City Information Desk at (303) 776-6050, Monday - Friday from 8 a.m. - 5 p.m.
